How to request that an Official License Verification be sent by the Board to another state

An official license verification must be requested through the eLicense Ohio portal (https://elicense.ohio.gov).

1. Login /Create an account

2. Go to your dashboard

3. Hit options under the bottom right hand corner of  your license tile

4. Choose send verification. (upload your states form if required)

Enter the requested information and submit your request.  The Ohio Speech and Hearing Professionals Board does not charge a fee for this request. 

Please login to your eLicense Ohio account to check on the status of your request.  Please allow approximately 7‐10 business days to process your request.

The following documents list the formal board actions taken by the Board during the given month. At the end of the calendar year, the monthly reports will be consolidated into a single document listing all actions taken by the Board in the year.

The Licensee Profile and Status featured on this website is a tool you can use to find or confirm a licensee with the Speech-Language Pathology and Audiology Board. 

To look up a licensee, follow these steps: 

On the Ohio E-License Center page, select "Speech Pathology and Audiology" from the drop down box next to the word "Division.

In the first box next to "-or- Name (Last, First)," enter the last name of the individual you are looking up. Tab over to the next box and enter the individual's first name or initial. You may use either capital or lower case letters. If you are unsure about proper spelling, or want to broaden your search, enter the last name only, or try entering just the first few letters of the last name. Click on "Search." 

Choose and click on a highlighted name from the list of matches that will appear at the bottom of the page. (NOTE: If you can't find the name you are looking for, or no results are found, try the search again using a different spelling or entering only the first few letters of the last name.) When you click on a highlighted name, the Licensee Profile page will appear. This page includes biographical and professional information about the licensee, as well as details of any public disciplinary action that has been initiated and/or taken. 

NOTE: The lookup system can display a maximum of 100 possible matches in response to a search request. You can narrow your search, making it easier to find the profile you seek, by completing additional search fields to specify the licensee's city, specialty, etc. Please consider this on-line license status information as fulfilling the primary source requirement for verification of licensure.
